The path to personalized cancer treatment

In the largest study of its kind, researchers have profiled genetic changes in cancer with drug sensitivity in order to develop a personalised approach to cancer treatments. The study is published in Nature on Thursday 29 March 2012.

The team uncovered hundreds of associations between mutations in cancer genes and sensitivity to anticancer drugs. One of the key responses the team found was that cells from a childhood bone cancer, Ewing's sarcoma, respond to a drug that is currently used in the treatment of breast and ovarian cancers. The lowered toxicity of this treatment may mean it is a safer alternative therapy for children and young adults with this aggressive cancer.

There is an intimate relationship between the way a drug works and the genetic changes present in cancers. This study found that sensitivity to most anti-cancer drugs is influenced by mutations in cancer genes and establishes the utility of using large-scale studies to identify these associations and build them into improved patient treatment.

"Our key focus is to find how to use cancer therapeutics in the most effective way by correctly targeting patients that are most likely to respond to a specific therapy," explains Dr Mathew Garnett, first author from the Wellcome Trust Sanger Institute. "We studied how genetic changes in a panel of >600 cancer cell lines effects responses to 130 anti-cancer drugs, making it the largest study of this type to date."

The team identified biological markers of drug sensitivity to a broad range of cancer drugs. Most of the cancer genes analysed, including those that are not known directs targets of the drugs tested, were associated with either sensitivity or resistance to at least one of the drugs analysed.

"Our research has taken us down unknown paths to find associations that are completely novel," says Dr Cyril Benes, senior author from Massachusetts General Hospital Cancer Centre. "We have identified hundreds of associations, many of which we still don't fully understand. We identified a novel indication for the use of PARP inhibitors, anti-cancer drugs currently used to treat breast and ovarian cancers, for the treatment of Ewing's sarcoma."

Ewing's sarcoma is a cancer of children and young adults with a 15% five-year survival rate in patients where the cancer has spread or they have relapsed after chemotherapy. The use of PARP inhibitors could represent a new treatment option for Ewing's sarcoma patients and these compounds will now be tested in clinical trials to assess their therapeutic benefit.

"Advances in next-generation sequencing technologies are already being translated into the large-scale detection of cancer gene mutations in the clinic," says Dr Ultan McDermott, senior author from the Wellcome Trust Sanger Institute. "There is a compelling need to identify, in a systematic fashion, whether observed mutations affect the likelihood of a patient's response to a given drug treatment. We have therefore developed a unique online open-access resource for the research and medical community that can be used to optimize the clinical application of cancer drugs as well as the design of clinical trials of investigational compounds being developed as treatments."

The team hopes their open-access database will be an important resource for the cancer research community and which will ultimately lead to improved treatments for patients. This research program is a unique Wellcome Trust funded 5-year collaboration between The Cancer Genome Project at the Wellcome Trust Sanger Institute and the Center for Molecular Therapeutics, Massachusetts General Hospital Cancer Center.

"Our work is helping to move cancer therapeutics away from the conventional tissue-based treatment to a more molecular-based treatment," says Professor Daniel Haber, senior author from Massachusetts General Hospital Cancer Centre. "The next steps for this collaborative project are to evaluate some of the key findings using tumour samples and test new candidate therapeutic strategies in clinical trials so we can hopefully improve the way we treat cancer patients. We are continuing our screening effort, in particular using drug combinations to discover innovative and better therapeutic options."

SugarSync 3.0 hits the iPad with revamped interface

Cloud storage provider SugarSync has already rolled out an updated version of its Android app this month, but it's now already back again with a major new release for iPad users. That comes in the form of the SugarSync 3.0 universal app, but the updates here are all on the iPad side of things, including a completely revamped interface, added gesture support, and generally more effective use of the iPad's added screen real estate. As before, the app itself remains free, but you'll have to pay up if you need more storage than the basic 5GB service provides ($5 a month will buy you 30GB, with plans up to 500GB available).

ROCHESTER, Minn. -- It's a natural laboratory for studying heart disease, lung problems, muscle loss, sleeping disorders and new medical technologies. It's also the highest mountain in the world. Mount Everest's extreme altitude puts climbers under the same conditions experienced by patients suffering from heart disease, obesity or advanced age. To take advantage of that, Mayo Clinic researchers are joining an expedition to Everest with National Geographic, The North Face and Montana State University. The Mayo group will monitor up to nine climbers from base camp for the duration of the climb, which will run from mid-April to mid-May.

"We can simulate some conditions in oxygen tents and hyperbaric chambers, but only for short periods," says Bruce Johnson, Ph.D., Mayo Clinic physiologist and leader of the scientific expedition. "We're studying the effects of extreme altitude on healthy, active individuals as well as these extreme athletes because what they experience mimics aspects of heart disease."

Dr. Johnson, who has conducted research at the South Pole and other mountain ranges, will be joined by three other Mayo investigators: physician-researcher Doug Summerfield, M.D., and scientists Bryan Taylor, Ph.D., and Amine Issa, Ph.D. Mayo Clinic also will send its own reporter to cover the research expedition. The Projects

Sleep Physiology

It's hard to get a good night's sleep in a cold tent, especially at 18,000 feet. Sleep apnea is common, and such starts and stops in breathing also trouble many overweight patients or those with chronic heart failure. Sleep apnea can decrease oxygen in the blood, a serious condition at sea level and at high altitude. Poor sleep quality occurs due to restricted space, high wind, hard surfaces; even clothing or equipment may play a role. Researchers will study oxygen during sleep and sleep quality as it relates to other high altitude problems.

Muscle Loss at High Altitude

Previous studies show that climbers lose weight rapidly at high altitude -- primarily muscle, not fat -- and extra food intake has negligible impact. Muscle wasting is a common problem with many chronic diseases, including heart disease. The hypothesis is that weight loss is related to more severe hypoxia, especially at night. The team will track calories, sleep quality, sleep hypoxia and body composition.

Lung Fluid Regulation

Pulmonary edema, fluid in the lungs common in heart patients, remains a mystery on mountains. Some think it's caused by an uneven constriction of blood vessels in the lungs, a response to the lower inspired gas pressures. Yet it doesn't happen to everyone who climbs, so there may be other factors, such as genetic susceptibility. Researchers will look at pressures in the lungs, gas transfer across the lungs and other factors.

Remote Monitoring Testing

Remote monitoring and recording devices will be tested, some for the first time under such extreme conditions. Ensuring they are rugged enough for Mount Everest and function under extreme cold and pressure changes will be valuable for future climbs and sea-level use. One monitor developed by Mayo researchers will get its first field test. If reliable data capture works on Mount Everest, quality should be assured in monitoring heart patients remotely in their homes, providing a tip-off when medication needs to be adjusted or a visit to the doctor is needed.

Research at Arizona State University and Columbia University to better understand the intricate sensory and cognitive connections between the brain and the hands has won support from the National Science Foundation. New discoveries about such connections could benefit people with neurological disorders such as Parkinson's disease and cerebral palsy, and those who need prosthetic hands.

The NSF has awarded a $640,000 grant to fund a research collaboration between Marco Santello, an ASU professor of biomedical engineering, and Columbia University scientist Andrew Gordon to expand their studies in this area.

Santello is also interim director of the School of Biological and Health Systems Engineering, one of ASU's Ira A. Fulton Schools of Engineering. Gordon is a professor of movement science in the Behavioral Science department in Columbia's Teachers College, where he coordinates the Kinesiology program.

The two have worked together for several years in pursuit of deeper knowledge about interactions between sensory feedback and motor actions involved in control of the hand.

In their current project they're seeking to determine the neural mechanisms that control learning and planning of the grasping and manipulation of objects.

They're examining the visual cues people use to assess object properties before they grasp or otherwise manipulate objects. In addition to using cues such as object shape or density, people often use memory of similar actions performed in the past.

"We can pick up these cues, and we can predict the result of our actions on an object, but we don't know exactly how the brain does this," Santello says.

Santello's focus is on neural control of the hand and the workings of senses such as vision and touch. By manipulating these senses in people with normal brain, hand and muscle functions, Santello studies what causes the performance of an action that is easy under normal conditions to become difficult. This allows him to identify what impact a specific sense has on how the brain controls the hands.

In his experiments, he imposes sensory deficits on test subjects, using goggles to block vision at selected times during the manipulation to interfere with the subjects' abilities to learn and execute grasping tasks. "We want to understand what aspects of visual feedback help the brain to successfully control grasping of an object and store a memory representation of that action," he explains.

Gordon's does cerebral palsy research. His related collaborative work with Santello focuses on cognitive aspects of the interaction between the brain and the hands, assessing the information the brain gains and processes from sensing the shapes of objects and exploring the role of memory of past actions.

"It's important to discern the basic mechanisms of storing the memory of recently performed actions, of planning future actions and integrating sensory feedback in healthy individuals, so that we might be better able to understand and treat neurological or musculoskeletal disorders," Santello says.

"In particular, understanding how we integrate 'what we know' from prior manipulations with an object and 'what we see' is crucial." Gordon says. "This is all the more important when our senses, and thus our ability to create sensory memories, is impaired, as is often the case in cerebral palsy."

Knowledge gained by such research can also be applied to improving neuroprosthetics. Current technology is able to provide extremely sophisticated artificial hands, but controlling the hands remains a challenge.

"The more we understand about the high- level processing that the brain has to go through to plan an action, the closer we will be to building more intelligent prosthetic systems that are capable of more human-like performance," Santello says.

Like Gordon, Santello has expertise in kinesiology the study of movement with a focus on kinematics of the hand, involving how the hand is shaped and how it performs grasping and related actions.

He began conducting hand research while working as a post-doctoral researcher at University of Minnesota, where he met Gordon, who had just completed his post-doctoral research there.

In the past eight years, they have expanded research into the workings of physical motor skills by incorporating recent advances in knowledge of biomechanics, neurophysiology and psychology.

Santello and Gordon now examine not only how objects are grasped, but look at why people choose to grasp an object in the ways they do.

By taking decision-making functions into account, they're trying to provide a more comprehensive view of the brain-hand relationship how, for instance, the brain and hand work together to create a memory of the position and force necessary to manipulate particular objects.

U.S. sets "surprisingly low" China solar panel duties

WASHINGTON/NEW YORK (Reuters) - The United States on Tuesday dealt a blow to U.S. manufacturers of solar panels and boosted shares in their Chinese rivals when it imposed preliminary punitive duties of less than 5 percent on imports from China.

Nonetheless, the action adds to trade tensions between the world's two largest economies and threatens cooperation in the burgeoning clean-energy sector, which both countries say they want to promote.

President Barack Obama, running for re-election in November, has promised to crack down on unfair Chinese trade practices and last week challenged China's export restrictions on critical "rare earth" industrial materials in a case filed with the European Union and Japan at the World Trade Organization.

Energy analysts had expected Chinese imports of solar panels to be hit with preliminary duties of 20 percent to 30 percent, but the rates announced on Tuesday ranged from just 2.90 percent to 4.73 percent.

Tuesday's trade move is the latest salvo from Washington in its efforts to help the nascent U.S. clean energy industry compete against China's fast-growing companies, which are the leading suppliers to the global solar market.

Rapid expansion by those Chinese companies has created a glut of solar panels that drove prices down sharply last year, pushing some weaker U.S. companies, including Solyndra, into bankruptcy.

Shares in the leading Chinese solar companies erased early losses to rally on the New York Stock Exchange on the news, with Suntech Power Holdings closing 14 percent higher, Yingli Green Energy gaining 12 percent and Trina Solar climbing 8 percent.

The Coalition for American Solar Manufacturing, a U.S. industry group that has complained that massive Chinese subsidies were driving them out of business, tried to put the best face on the news.

It said it believed the U.S. Commerce Department would uncover more subsidies and unfair pricing practices as it continues its probe in the coming months, which would result in higher final duties.

"Today's announcement affirms what U.S. manufacturers have long known: Chinese manufacturers have received unfair and WTO-illegal subsidies," said Steve Ostrenga, chief executive officer of Helios Solar Works in Milwaukee, Wisconsin.

Senator Ron Wyden, an Oregon Democrat who has been a driving force behind the case, also said he expected duties to "significantly swell" as the case proceeds.

An attorney representing SolarWorld Industries America and other U.S. manufacturers said Commerce Department officials have only just begun to investigate some of the group's subsidy charges, providing hope duties would increase.

China accounts for 114 of the 283 anti-dumping and countervailing duty orders the United States has on foreign goods. The Obama administration has imposed more than 50 anti-dumping and countervailing duty orders since taking office in January 2009, including about 40 against Chinese goods.

The United States imported $2.8 billion worth of solar cells and panels from China in 2011, up sharply from about $1.2 billion just a year earlier, according to industry estimates.

The Commerce Department will announce preliminary anti-dumping duties in May to address a separate set of charges that Chinese producers are selling solar panels in the U.S. market at unfairly low prices.

CHINESE COMPANIES FEEL 'VINDICATED'

Chinese producers and U.S. companies opposed to the duties said the preliminary decision on Tuesday belied charges that China was flooding its solar sector with subsidies.

"We're pleased and in large part feel vindicated," Robert Petrina, managing director of Yingli Green Energy's U.S. business said. "I think it is a positive outcome and I think it really speaks to the crux of the argument."

Jigar Shah, president of a coalition of U.S. solar panel sales and installation companies which were opposed to duties, called the ruling an "initial victory for America's solar industry and its 100,000 employees" because it would not significantly raise prices for solar products and hurt demand.

Rob Stone, a solar analyst with financial services firm Cowen & Co, said the duties, however small, push solar panel prices in the wrong direction if the sector wants to become a viable alternative to fossil fuels.

"In the end, solar needs prices to go down more from where they are now, even though they've come down precipitously," Stone said. "This is why this whole thing is so stupid, because it will not help the small, uncompetitive U.S.-based manufacturers."

SolarWorld Industries America, the U.S. arm of leading German solar manufacturer SolarWorld AG, has led the U.S. industry coalition seeking import relief.

The Commerce Department set a preliminary duty of 2.90 percent on SunTech Power Holdings, the world's biggest producer of photovoltaic solar panels, and a preliminary duty of 4.73 percent on Trina Solar, another major Chinese producer, industry officials said. All other Chinese solar panel producers and exporters received a duty rate of 3.59 percent.

Importers will have to post bonds or cash deposits based on the preliminary countervailing duty rates while the department continues its investigation.

The "surprisingly low" numbers would likely mean that the major Chinese companies would need to pay between about $5 million to $10 million to cover products shipped, said Timothy Arcuri, an analyst with Citigroup.

While the dumping duty decision in May could impose a greater penalty on the Chinese companies, "this by itself has relatively minimal impact," Arcuri said.

When it comes to healthy eating, many people follow the sage advice of, ?everything in moderation.? I generally follow this advice myself.

Yet, a pair of new studies from Harvard School of Public Health show that moderate consumption of red meat and soft drinks can still significantly raise your risk of death, especially from heart disease.

When researchers studied the nutritional habits and health outcomes of thousands of men and women over more than twenty years, they learned that:

  • One daily 3 ounce serving of red meat raised risk of death by 13 percent
  • One daily serving of processed red meat (such as a hot dog or 2 slices of bacon) raised risk of death by 20 percent
  • One 12-ounce sugar-sweetened beverage (such as most soft drinks) raised risk of heart disease by 20 percent. Soft drinks also raised triglycerides, a harmful type of fat in the blood.

Those are scary stats. What to do? Here are some tips.

Switch to diet soft drinks. Ideally, you wouldn?t drink soft drinks at all. But there?s a difference between what?s ideal and what?s realistic. If you can?t give them up, at least switch to diet. Artificially sweetened drinks were not linked to an increased risk of heart disease in this study. One better than diet drinks, however, is water. Flavor it with lemon, lime or a splash of juice. Or consider herbal tea.

Cut back to a half serving of red meat a day. That?s roughly the size of half a deck of cards. That's a very small serving. Use it as an accent for a meal that is mostly composed of vegetables, whole grains or legumes.

Eat another source of protein instead. People who ate fish, chicken, nuts, legumes, or lowfat dairy instead of red meat dropped their risk of death considerably. Nuts had the strongest effect, dropping risk of death by 19 percent.

Departing Goldman banker slams 'rip-off' culture

LONDON/NEW YORK (Reuters) - Goldman Sachs faced an unprecedented assault from one of its own on Wednesday after a banker published a withering resignation letter in the New York Times, calling the Wall Street titan a "toxic" place where managing directors referred to their own clients as "muppets."

It was the latest blow for the investment bank. The company -- dubbed a "great vampire squid" in a 2009 article in Rolling Stone magazine -- has been embroiled in the biggest-ever insider trading scandal on Wall Street. And just weeks ago, a top judge criticized Goldman for big conflicts of interest in an energy deal.

In an opinion column in Wednesday's Times, Greg Smith, who worked in equity derivatives, said Goldman had become "as toxic and destructive as I have ever seen it.

"It makes me ill how callously people talk about ripping their clients off. Over the last 12 months I have seen five different managing directors refer to their own clients as 'muppets,'" Smith said.

In the United States "muppet" brings to mind lovable puppets like Kermit the Frog, but in Britain, "muppet" is slang for a stupid person. (Goldman, as it happens, was at one time also the bank for the family of Muppets creator Jim Henson.)

Goldman Sachs issued a short statement in response:

"We disagree with the views expressed, which we don't think reflect the way we run our business.?In our view, we will only be successful if our clients are successful.?This fundamental truth lies at the heart of how we conduct ourselves."

In a memo to staff, Goldman Chief Executive Lloyd Blankfein and Chief Operating Officer Gary Cohn said Smith's views were in the minority among his 12,000 fellow vice presidents.

"And, what do our people think about how we interact with our clients??Across the firm at all levels, 89 percent of you said that the firm provides exceptional service to them," they said in the memo, a copy of which was reviewed by Reuters.

Congressman Barney Frank, an architect of the 2010 Dodd-Frank financial reform law, said Smith's piece would have "a big impact" on the banking industry's efforts to push back against financial reform.

"It puts the burden on Goldman Sachs and others to show us how what they do benefits the clients and therefore the broader economy," he told Reuters.

Goldman shares closed 3.3 percent lower, on a day when broader markets were up slightly. At least one bank wasted no time in trying to take advantage of the situation.

"In my experience ... client success and firm success can peacefully coexist; in fact thrive," Harris Private Bank Chief Investment Officer Jack Ablin said in an open letter.

"Having served clients for nearly 30 years I can tell you that the long-term success of any institution, whether in the financial field or not, depends on the long-term success and satisfaction of its clients," said Ablin, who oversees $60 billion of investments for individuals and families.

But the company, which sometimes lacks for defenders, garnered at least some public support in response to Smith.

"The many people we have dealt with there have all been exceptionally talented and high-grade, and never once have we had a negative experience in which we felt that they took advantage of us or didn't do what they said they would do," well-known fund manager Whitney Tilson said in a note.

FRIENDLY AND GENUINE?

Smith, who did not return voice mails on his cellphone, carried the title of executive director, but it was not nearly as illustrious as it might sound. Goldman's roughly 12,000 vice presidents and executive directors compare with 450 managing directors -- the next rung up in the Goldman hierarchy and a job classification that Smith didn't achieve. Overall, the company has about 33,000 employees, meaning that 36 percent of Goldman's workforce carried a title similar to Smith's.

South African David Berman, founder of hedge fund Durban Capital and friends with Smith, called him "a very understated, humble type of guy who would tell the truth as truth is important to him.

"But he never struck me as the Goldman-type as he isn't aggressive nor (the) salesman type one expects," Berman said, adding, "I am convinced this is for real with no selfish intent here."

According to the British Financial Services Authority's register, Smith joined Goldman's UK unit a year ago.

Johannesburg-born Smith attended universities in his home country and in the United States, where he received a degree in economics from Stanford University in 2001. He also interviewed to be a Rhodes Scholar in South Africa in 2002.

While at student at Stanford he had a summer internship at Paine Webber in 1999 and a summer internship at Goldman in 2000. Upon graduating from Stanford in 2001, he landed at Goldman.

Internally, Smith's op-ed piece was not necessarily well received. A trader, who knew Smith in passing, said the company is telling staff that Smith is a disgruntled employee who is leaving because he didn't make managing director.

This trader, who did not want to be named, says former Goldman colleagues are saying that Smith "wasn't very commercial," which means he wasn't producing the kind of sales the company wanted.

PAST MEDIA STORMS

Goldman Sachs -- fourth among investment banks last year based on fee-income rankings compiled by Thomson Reuters and Freeman Consulting -- has a history of tension with client interests, experts say.

"Greg Smith refers to the last 12 years, but in fact Goldman has been doing this kind of thing since going back to the Great Depression," author William Cohan told Reuters Insider.

"It's not just the last 12 years; unfortunately it's part of the firm's DNA," said Cohan, author of the Goldman profile "Money and Power" and a former Wall Street banker himself.

In recent years the company has faced other high-profile incidents damaging to its image after the near-collapse of the global banking system in 2008.

Earlier this month it was accused of a major conflict of interest for advising El Paso Corp on its sale to Kinder Morgan, while being a significant shareholder in Kinder.

A lawyer representing an Australian fund in a lawsuit against Goldman over mortgage-backed securities, filed in New York last year and alleging fraud and breach of contract, said he may seek Smith's deposition to help bolster his case.

"Part of Goldman's defense is everybody is sophisticated and everybody knew as much as we knew did," the lawyer, Eric Lewis, said. "But if you're calling your clients muppets -- most muppets don't have the cranial capacity of Goldman."

Paul Volcker, a former Federal Reserve chairman, called the Smith piece a "reflection of the change in market mentality over the last 15, over the last 20 years" at an economics summit?in Washington hosted by the Atlantic magazine.

TWITTER IGNITES

Unsurprisingly, Smith's resignation letter captured the imagination of Twitter users. "Greg Smith" was a worldwide trending topic early Wednesday, meaning it had suddenly spiked in interest, while both that and "Goldman Sachs" were trending in the United States.

Many of the commentators expressed surprise about the allegations in the piece, while others called for Smith to shed light on why he left the bank, or pointed out that he seemed to have been employed in a comparatively junior role.

As happens on the Internet in cases like this, near-instant parodies of Smith's letter cropped up. The most popular by far had Darth Vader of "Star Wars" fame resigning from the Empire via a letter similar to Smith's.

"To put the problem in the simplest terms, throttling people with your mind continues to be sidelined in the way the firm operates and thinks about making people dead," the film franchise's dark lord wrote.
